PhysicalsAll scouts and adult leaders at Hale must
have a completed Boy Scouts of America physical examination form on file
at the Health Lodge. Adults
older than 40 years of age must have a physical examination each year and
use the Personal Health and Medical Record Form Class 3 (No.334112).
Scouts and adults under 40 use the Class 2 physical examination form (No.
33414). If a scout or adult
under 40 has had a physical examination within the past 36 months, they
can attach a copy of that physical to a new No. 33414 and only need to
complete the Class 1 Personal Health and Medical History portion.
ALL physical forms for minors MUST be signed by the parent of
guardian. Adults MUST sign
their own physical. Physical
forms will be returned to the troop at checkout. A copy of the physical form is included
with guide. Physical forms are
also available at the Indian Nations Council located at 4295 S. Garnett
Road, Tulsa Oklahoma 74146 (918-743-6125). We strongly recommend that all physical
forms be collected by the troop’s adult leader two weeks prior to camp
to ensure that all scouts have a properly completed form. There is no charge for medical services
performed in camp for scouts and leaders, except for certain medical
supplies. All campers needing
services performed at the hospital/clinic are required to handle payments
through the troop of family insurance policy. Although the Health Lodge is open 24 hours
a day, please limit non-emergency visits to those times in the morning and
evening dedicated for sick call.
